Our nurses and social work case managers can help you with:
- Transition planning, whether you are leaving the hospital to return home or are moving on to an acute care or skilled nursing facility
- Connecting you with community resources and benefits programs
- Working closely with physicians and other team members to ensure that your care is appropriately reimbursed
- Interpretation and translations services
Our team consists of nurses, social workers utilization review assistants and a case aide who have expertise in a broad range of health care-related issues.
Our case managers are here to help you and your family cope with your illness or injury. They can provide emotional support, counseling, and referrals to community resources for language interpretation, financial counseling and transportation needs.
