Care Management

Care Management at UVM Health

A care manager offers various types of support. They provide personalized assistance to help you manage your health and educate you on how to take care of yourself. They also help you address challenges that might prevent you from staying healthy, such as transportation or financial issues. Essentially, a care manager acts as a partner, guiding you to maintain your health and navigate the health care system.

Care management is integrated into University of Vermont Health primary care practices across Vermont and northern New York, making it more convenient to access our services. Our team will work with you to understand the obstacles in your way and identify strategies to overcome them, so you can focus on staying as healthy as possible.

What is Care Management?

Care management team members work closely with you and your health care providers to help you manage your health issues, follow your care plan and maintain your overall well-being.  

Your care management team coordinates your care to ensure your physical and mental health needs are being met. They can help you: 

  • Improve your health and quality of life 
  • Take care of your long-term health concerns and conditions 
  • Communicate with your primary care provider and specialists 
  • Find community resources to support you and your family

Our Services

Any patient with a UVM Health primary care provider is eligible for care management services, from children to older adults.

Our multidisciplinary care management team has decades of experience working with physicians, clinicians and nurses to help patients optimize their health. We support each of our patients with a team built around their unique needs, which may include:

  • Primary care providers
  • Family and caregivers
  • Care managers
  • Pharmacists
  • Specialists and other providers

We take a patient-centered approach to care management, meaning you are the most important member of your team. We’ll work closely with you to understand your health challenges, set realistic goals and create a plan to help you meet them.

Support Services

Our team can also connect you with additional support services within UVM Health or in your community, including:

  • Food assistance
  • Housing assistance
  • Prescription assistance
  • Social work services

If you need help managing your health, please speak with your primary care provider to be connected to the care management team. We offer both digital health visits and in-person appointments at primary care locations across UVM Health.
