Care Management
Care Management at UVM Health
A care manager offers various types of support. They provide personalized assistance to help you manage your health and educate you on how to take care of yourself. They also help you address challenges that might prevent you from staying healthy, such as transportation or financial issues. Essentially, a care manager acts as a partner, guiding you to maintain your health and navigate the health care system.
Care management is integrated into University of Vermont Health primary care practices across Vermont and northern New York, making it more convenient to access our services. Our team will work with you to understand the obstacles in your way and identify strategies to overcome them, so you can focus on staying as healthy as possible.
What is Care Management?
Care management team members work closely with you and your health care providers to help you manage your health issues, follow your care plan and maintain your overall well-being.
Your care management team coordinates your care to ensure your physical and mental health needs are being met. They can help you:
- Improve your health and quality of life
- Take care of your long-term health concerns and conditions
- Communicate with your primary care provider and specialists
- Find community resources to support you and your family
It made a big difference having another person available for me to use as a sounding board, cheerleader, and just someone who could understand the process I was going through. I highly recommend this program.
