Within MyChart, you can self-register for emergency services at some emergency and urgent care locations.

Self-Register for Emergency Services

We currently offer self-registration at all University of Vermont Health emergency department locations and Fanny Allen Urgent Care.

How do I complete self-registration while in the emergency room?

Patients arriving at the emergency department (ED) or urgent care may choose to receive a link via email or text message to complete self-registration through MyChart. You do not need a MyChart account to use this self-service feature.

Can someone else complete self-registration for me?

Yes. You may have someone with you use your smartphone to complete the registration on your behalf.

What happens to the link if I don’t complete self-registration or leave the ED before being seen?

The link is single-use and only active during your current ED visit. Once used, or after you leave the ED, the link will no longer be available.

I did not receive the link, can I still self-register?

If you need the link resent, please check in with the front desk or your care team to have it sent again. If you are an active MyChart user, you can log in to your account and complete the self-registration process under the "Happening Now" section for your current visit. If you do not have a MyChart account, you can sign up here.

How is my registration information saved?

Your registration information is saved each time you click on “Save,” “Next,” or “Submit.” Staff is able to see which steps have been completed and which still need attention.

I need help. What should I do?

If you need help, please ask a registration staff member. If you can’t finish the registration online, registration staff and your care team will assist you.

Will using the self-registration feature help me see a provider sooner?

Patients are seen based on the severity of their condition. Self-registration may reduce time spent with registration but does not affect your care or how quickly you are seen by the care team.

What happens after I complete registration?

A confirmation screen will appear once your registration is complete. If any information is missing or additional details are needed, a registration staff member will follow up to complete your registration.
