Getting the Support You Need

Managing your health and knowing where to go for support can be hard, and that’s where the Care Management team can help. Our multidisciplinary team connects with patients through their primary care providers. If you are having trouble managing health issues and well-being or following your care plan, you can ask your provider if you might benefit from Care Management support. Your provider may also refer you to our services.

Any person who has a primary care provider in the UVM Health Network system is eligible for care management services. If you are not already connected to a provider, we’ll help you find one.

Your care manager can connect you with support services and programs across UVM Health Network, as well as services in your community that the Network may not provide.

It made a big difference having another person available for me to use as a sounding board, cheerleader, mentor and just someone who could understand the process I was going through. I would highly recommend this program and hope it continues so that it can help other people.

The Care Management Team

The Care Management team has decades of experience working closely with physicians, clinicians and nurses in primary care practices across UVM Health Network. Your team will be tailored to your needs, but will include, among others:

  • You
  • Your health care provider(s)
  • Your family/caregiver (if there is one)
  • Care managers
  • Pharmacists
  • Other specialists and providers of care

We will work with you to set goals and make plans to meet them. Our priorities are to support you in:

  • Improving your health and quality of life
  • Taking care of your long-term health concerns and conditions
  • Working closely with your primary care provider and specialists
  • Finding community resources to support you and your family

We are here to provide the best possible care, without judgment.
