Family Medicine
Family Medicine at UVM Health
Family medicine providers at University of Vermont Health specialize in complete care for every member of your family. Our services cover patients from infancy through the senior years, as well as pregnancy care.
Multiple generations of families come to us for our compassion and our expertise in preventive care, chronic disease management and urgent medical needs. We partner with your family on customized care plans that ensure every member of the family benefits from good health.
Why Choose UVM Health?
We offer:
- Family-centered care: Your family benefits from seeing the same provider at the same practice throughout the years. You build a long-term relationship with someone who knows your family and understands your unique needs.
- Chronic disease management: We know managing chronic conditions like diabetes, asthma or heart disease can be challenging. Our Care Management team offers support to help address barriers to care and lower the risk of complications and hospitalization.
- Mental health support: Mental health and physical health are equally important. That’s why mental health care is part of family medicine services. Your provider may prescribe medications, manage mental health concerns or refer you to a specialist for higher-level care.
- Care founded in research: Our providers use the best evidence to guide your care and treatment. Many of our providers are active in research to improve primary care services, including the Northern New England Clinical and Translational Research Network (NNE-CTR).
