Inpatient Care Referral
The Transfer Center is available 24 hours a day to facilitate the transfer of adult and pediatric patients to UVM Health hospitals.

Adult Outpatient Care Referral
All referral forms can be sent to us at PatientAccessCenter@uvmhealth.org or by fax at 802-847-2001.

Home Health & Hospice Referral
To refer a patient to Home Health & Hospice, please complete the below form(s):
You can also call 802-860-4400 or 1-800-427-1908. Our intake and referral team is available Monday to Friday, 8 am–5 pm.
If your patient has an urgent medical need outside of business hours, please call 1-800-427-1908 to speak to the nurse on call.
For non-urgent requests outside of business hours, please leave a message on our confidential voicemail system at 802-860-4484. An intake nurse will return your call on the next business day.
Completed referral forms can be faxed to 802-860-4464.
Please note that Home Health & Hospice only serves residents of Chittenden and Grand Isle Counties, except for the McClure Miller Respite House which has no residency restrictions.
Using Direct Addresses to Send Patient Information to UVM Health EHR
If you're part of a health care organization, you can securely send patient information directly to our electronic health record (EHR) system using a Direct Address.
A Direct Address looks like an email address (e.g. provider@direct.example.com), but it’s not used like regular email. Instead, Direct Address is part of a secure network designed specifically for exchanging health information between EHR systems.
To use a Direct Address:
- Your EHR system must be set up to send messages using Direct messaging.
- You’ll need to work with your EHR vendor to configure this feature.
- Once set up, you can send patient records directly to our EHR using the Direct Address listed below.
This method helps ensure patient information is delivered securely and appears in the EHR quickly — making it easier for providers to access and act on it.
