About Our Location
The Au Sable Forks Health Center at University of Vermont Health – Elizabethtown Community Hospital is a primary care clinic. Our primary care providers perform routine check-ups, well-care appointments and coordinate care and referrals to specialists.
Our providers partner with you, listen to your concerns and get to know you before developing care plans that meet your unique health needs.
In the event that our clinic staff cannot treat a certain condition at the center, they will provide referrals to services within University of Vermont Health.
