Request Your Medical Records

To request any portion of your medical record, please print and complete the authorization to release patient information form and fax, mail or bring it to your physician's office.

  • Most people just need a portion of their record, though you can get your whole record if needed. Please note on the form exactly what information you need.
  • We can only release information once your doctor has signed off on it and it's considered final - unless you're sending the information to another doctor for your continued care.
  • A flat fee of $5 will be charged for the first 10 pages; for every page over 10, the charge is .50 cents per page. The fee is waived for copies sent directly to a healthcare provider or facility for continuing medical care.

Send your records to another provider

On the authorization form you can indicate where you want your records sent. Records sent to another healthcare provider or facility are not subject to any fees.

Request Radiology Images

To request copies of your radiology images, such as X-rays, CT scans or MRI films, contact the Imaging Library at 802-847-2354.

Request a Birth or Death Certificate

If you need to get either a birth or a death certificate, please contact Burlington City Hall at 802-865-7000.

Correcting Your Record

If you think the information in your medical record is incorrect, you can complete and submit the Request for Correction/Amendment of Protected Health Information form and submit it to the Health Information Management Department at the mailing address or fax number provided on the form.

Who Controls Your record?

Your record is the physical property of the hospital. However, the patient controls the release of the information contained in the record. In general, you must give permission for anyone, other than a member of your healthcare team, to have access to your medical record. By law, your records may be disclosed without your permission under certain circumstances such as in response to a subpoena or court order, to certain government and regulatory bodies, to another healthcare provider for continued care, and to your health care insurer to obtain reimbursement for your care.
