Working together to strengthen the health of our communities.

Community Benefits are programs and activities designed to address and improve community health priorities.

These efforts include subsidizing health services, providing health education to the community, investing time and money in our communities and more. As part of IRS requirements, CHI tracks and reports these activities, which last year equaled $432,919,208 for FY 23.

What is a Community Health Needs Assessment?

Every three years, each UVM Health hospital conducts a Community Health Needs Assessment (CHNA) in collaboration with key community partners. These assessments help us:

  • Understand the health challenges and opportunities in our communities.
  • Identify top priorities for action.
  • Foster meaningful dialogue and goal setting.

CHNAs guide our work — not as exhaustive studies, but as tools for collective action toward healthier communities.

Each CHNA is followed by a Community Health Improvement Plans (CHIPs) or Community Service Plans (CSPs) — three-year roadmaps that outline how we'll address the community’s top health priorities. These plans combine existing programs with new initiatives to meet community needs.

University of Vermont Medical Center

Every three years, University of Vermont Medical Center conducts a Community Health Needs Assessment (CHNA). This process helps us to better understand the challenges and opportunities involved in creating a healthier community.

The CHNA gathers information in a variety of ways, with the goal of reflecting the views of community members. This part of the process ends with a consensus on priority community health needs. The results then inform an implementation strategy, or Community Health Improvement Plan, which outlines how priority health needs will be addressed for the following three years by both the hospital and our community partners.

University of Vermont Medical Center's 2022 Community Health Needs Assessment covered our Health Service Area of Chittenden and Grand Isle counties. The 2022 CHNA process was the most inclusive and robust yet in our history. We are proud to have collaborated with over 25 community partners on our CHNA Steering Committee, including:

2022 CHNA Vision and Goals

The 2022 CHNA process will guide measurable community health improvements in our Health Service Area. The goals of this process are:

  1. To conduct, through the lens of racial and health equity, an inclusive, high-quality assessment of community health needs and assets in Chittenden and Grand Isle counties.
  2. To partner with diverse stakeholders, resulting in:
  • Consensus of priority needs to address
  • Shared buy-in for implementation strategies
  • Support of complementary community initiatives and assessments

Data Gathering and Community Health Priorities

We collected data through interviews, surveys, and focus groups held in our local community, in addition to reviewing 70+ health indicators available for the two counties. Two virtual sessions were held in January 2022 to engage community leaders and champions in prioritizing six health priorities that emerged from the data gathering phase.

The top three community health priorities, in alphabetical order:

  • Cultural Humility and Inclusive Health Care
  • Housing
  • Mental Health and Wellbeing

The CHNA findings will inform the 2023-2025 Community Health Improvement Plan (CHIP) that University of Vermont Medical Center will develop in collaboration with key partners.

The CHIP process will:

  • Facilitate inclusive community engagement to generate solutions that build upon strengths and address inequities.
  • Communicate shared goals, strategies and resources to make measurable improvements.

Central Vermont Medical Center

Central Vermont Medical Center works closely with other regional health providers to meet the health care needs for Central Vermonters. To ensure these services are aligned with the health care needs of our community, we conduct a Community Health Needs Assessment (CHNA) every three years. This study helps us better serve our community by measuring the health status of residents, gathering wide community input on health concerns, and identifying opportunities to collaborate with partners.

The CHNA results and related action plan guide our community health activities over a three-year cycle.

These reports cover a large range of topics but are not a complete analysis of any one issue. Rather, these data help to identify priorities which lead to productive community discussion and the creation of goals. We invite the reader to investigate and use the information in this report to move toward solutions for healthier communities.

CHNA Participation

The 2022 CHNA was conducted in collaboration with THRIVE, the regional Accountable Community for Health. This multi-agency coalition, made up of health providers, social service agencies, government, civic and religious entities, and numerous other community partners, is dedicated to improving health for the residents of Washington and Northern Orange Counties. THRIVE members played an integral role in overseeing data collection and reviewing findings to determine community health priorities based on the CHNA study.

In addition to THRIVE members, more than 1,500 community residents shared their perspectives on community needs through surveys and open dialogue. We value this feedback and recognize that all community stakeholders play an integral part in advancing the health of Central Vermont.
