Home Health & Hospice plays a valuable role in the lives of the residents of Chittenden and Grand Isle Counties. Over 120 years we’ve served our neighbors at every age and stage of life, from pregnancy and early childhood to adulthood and the end of life.

We depend on your donations to help us fulfill our mission of providing high-value, compassionate care wherever people call home. With your support, we’re able to continue providing services that enrich the lives of our patients and their families and caregivers.

Home Health & Hospice Fund

The Home Health & Hospice Fund provides flexible, essential support to drive our mission, helping us meet the evolving needs of patients and families. Your gift to this fund is directed to the areas of the organization that need it most.

Family and Children's Fund

Gifts to this fund allow us to meet the needs of children and families in our community, including:

  • Nursing and family support: Provides whole-family care to aid children’s healthy development.
  • Pediatric rehabilitation: Includes in-home physical, occupational and speech-language therapies

Adult Home Health Fund

Our Adult Home Health program offers a wide range of services for adults with short- and long-term conditions. Your support:

  • Helps those in need heal safely at home and avoid unnecessary hospital visits, making sure our neighbors receive the care in the best setting.
  • Powers innovative programs like High-Tech care, Longitudinal Care and Remote Patient Monitoring for neighbors with some of the most challenging health conditions.

Adult Day Program Fund

This community-based daytime program is for older adults and individuals with disabilities and Alzheimer’s disease or other memory impairments. Donations to this fund support:

  • The purchase of ingredients and activity materials that keep participants nourished, engaged and connected.
  • Nursing and personal care services to enhance the health and independence of participants.

Long-Term Care Program Fund

This federally supported program helps older adults and adults with disabilities remain in their own homes. Your donations supplement our services, which include:

  • Assistance for personal care and household tasks so people remain safe and independent at home

  • Regular companionship that steadies care and prevents avoidable setbacks.

Hospice and Palliative Care Fund

Our Hospice & Palliative Care program cares for people living with terminal illnesses and at the end of life, whether at home or at the McClure Miller Respite House. Your gift:

  • Provides expert, compassionate hospice and palliative care for people in their final months, days and hours, as well as for people living with life-altering illnesses.
  • Provides supportive and bereavement services for families.
  • Helps train the incredible volunteers committed to being present during one of life’s most difficult moments.

McClure Miller Respite House Fund

Donations support Vermont’s only Medicare-certified inpatient hospice residence, providing exceptional end-of-life care in a homelike setting. Your donation supports:

  • Access and affordability for the more than 300 families that utilize the Respite House each year, ensuring that no one is turned away due to ability to pay.
  • Support for families through services to help loved ones be present, informed, and at ease.

Workforce Development Fund

We are dedicated to helping our employees gain education, skills and professional certifications to advance their careers. With your support, we can:

  • Offer scholarships and fund training and certifications that raise quality across our programs.
  • Provide career pathways that help learners fill the most needed health care roles.
