Home Health & Hospice plays a valuable role in the lives of the residents of Chittenden and Grand Isle Counties. For more than 100 years, we’ve served our neighbors at every age and stage of life, from pregnancy and early childhood to adulthood and the end of life.
We depend on your donations to help us fulfill our mission of providing high-value, compassionate care wherever people call home. With your support, we’re able to continue providing services that enrich the lives of our patients and their families and caregivers.
Our Priority Initiatives
To help us sustain our critical programs and services, we’ve identified where your gifts would have the greatest impact. You can also contact us to direct your gift to an area of specific interest.

The Home Health & Hospice Fund touches every aspect of our mission and helps us improve the health and well-being of our patients and their families. Your gift to this fund will be used to support areas of the organization that are in most need of assistance.
The McClure Miller Respite House is the only Medicare-certified inpatient hospice residence in Vermont. Donations to this fund help us provide exceptional end-of-life care in a homelike atmosphere.
Our Adult Home Health program offers a wide range of services for adults with short- and long-term conditions. Services we provide include:
- Skilled nursing
- Medical social work
- Physical, occupational and speech-language therapy
- Remote patient monitoring
- Wound care
With your support, we help people recover from illness and injury and enhance their ability to live independently.
This fund supports the care of people living with terminal illnesses and at the end of life. Our interdisciplinary teams use a whole-person approach, focusing on alleviating pain and promoting dignity and quality of life. We also provide emotional and spiritual support for patients and their families. Our bereavement team works with families after the passing of a loved one to help process their loss.
Gifts to this fund allow us to meet the needs of children and families in our community, including:
- Nursing and family support: Provides whole-family care to aid children’s healthy development.
- Pediatric rehabilitation: Includes in-home physical, occupational and speech-language therapies

This community-based daytime program is for older adults and individuals with physical disabilities and memory impairments. Our caring staff provide nursing and personal care, nutritious meals and fun activities. Donations to this fund support day-to-day operations and help us make sure our facility is well-equipped.
This federally supported program helps older adults and adults with disabilities remain in their own homes. Your donations supplement our services, which include case management, personal care, help with household tasks and companionship.
