We provide a range of assistance programs in the comfort of home for:

Family Home Care Services
We provide a range of home care services for patients and their families depending upon your needs.
Pregnancy, the postpartum period and early childhood years can be stressful times. Our caregivers work with families to support healthy child development and address stressors such as physical and mental health issues, food insecurity and housing instability.
Our family-based services are designed to identify your family’s strengths and build from there. This positive approach helps your family use your existing resources and resilience to improve your well-being. Our services may include:
- Childbirth preparation and breastfeeding support
- Guidance to help young children develop age-appropriate skills
- Pregnancy and postpartum care for people with high-risk pregnancies due to medical, psychiatric or substance use disorder diagnoses
- Self-care and parenting assessments, and assistance in addressing any identified issues
Managing care for a child with a chronic condition or developmental delay can be challenging. We offer in-home nursing care, family support and rehabilitation therapy (including physical, occupational and speech-language therapy) to promote your child’s healthy development.
Our therapists provide assessments and interventions for children who have, or are at risk for, developmental delays, and those with acute and chronic conditions. To engage with children and their families as early as possible, we work closely with primary care providers and Vermont Children’s Integrated Services programs, including Early Intervention Services and Strong Families Vermont Home Visiting Program.
Our in-home providers also provide resources to support the entire family, including wellness and prevention services and education on how to best meet your child’s needs.
We provide in-home nursing care for children and young adults up to age 21 who are enrolled in Vermont Pediatric High Tech Nursing. These services are for children with complex medical needs who require ongoing nursing care to maintain their health and safety.
Under the supervision of a health care provider, our registered nurses (RNs) and licensed practical nurses (LPNs) implement your child’s care plan and manage their medical supplies and equipment as needed. Our support helps children remain at home and gives caregivers the opportunity to rest or take time for themselves.
A child’s serious medical condition can impact your entire family. Our Pediatric Palliative Care Program can help you care for your child and make life a little easier.
We offer palliative care services for families of children up to age 21, including:
- Education about your child’s illness and their medical care, such as how to give medications and use medical equipment
- Expressive therapies, such as music, art and dance movement therapy, to help your child process the mental and emotional aspects of their condition
- Help accessing community resources that can benefit your child and family
- Bereavement services for children and families
How to Access Family Home Care
Most of our programs have specific insurance and clinical criteria. You may need to ask your physician for a referral or apply through the State. You must also receive services in Chittenden or Grand Isle County.
