Helping Hands for Older Adults and People with Chronic Conditions

Caring for a family member can be challenging, stressful — and rewarding. University of Vermont - Home Health & Hospice offers support for families in Chittenden and Grand Isle counties caring for older adults and people with disabilities and chronic medical conditions. Supportive Care Services

We provide a range of services to support our patients and families depending upon your needs.

Adult Day Program

Located in South Burlington, this daytime program serves older clients, adults with disabilities and individuals with Alzheimer’s disease or other memory impairments. We’re not just another program — we’re a family of caregivers who care deeply for our clients.

Our Adult Day Program provides respite for caregivers and enhances the health and independence of participants through:

  • Engaging activities that promote social connections
  • Meals and snacks that meet 100% of our client’s nutritional needs
  • Nursing and personal care services
Longitudinal Care Program

This program is for clients 18 and older who have completed care through our Home Health Program and are at high risk of rehospitalization or further health problems. Our team of nurses, community health workers and social workers offers in-home wraparound services to help clients:

  • Manage health conditions and coordinate care
  • Monitor and detect new medical concerns to allow for early intervention and treatment
  • Overcome barriers to health and wellness, such as food insecurity or lack of transportation
Remote Patient Monitoring Program

Remote patient monitoring allows providers to monitor your health data outside of a traditional health care setting, so you can manage your health from home. Your health data is collected through an easy-to-use device and transmitted wirelessly to your provider. They review your vital signs and symptoms daily and contact you to discuss changes as needed. This type of monitoring gives patients and their families peace of mind and may prevent further hospitalizations.

Long-Term Care Program (Vermont Choices for Care)

We provide caregiver services for people enrolled in the Vermont Choices for Care Program. Under Choices for Care, participants can receive a variety of nonmedical services to support their independence, well-being and safety at home. These services may include help with:

  • Bathing
  • Dressing
  • Personal hygiene
  • Grocery shopping
  • Housekeeping
  • Laundry
High-Tech Nursing Program

We provide nursing care for adults enrolled in Vermont Adult High Technology Services. These services are for people 21 and older who live in home-based settings and require medical equipment, to maintain their health and safety.

Under the supervision of a health care provider, our registered nurses (RNs) and licensed practical nurses (LPNs) provide care and manage medical supplies and equipment as needed. Our support helps clients remain at home and gives caregivers the opportunity to rest or take time for themselves.

We also offer high-tech services for children.

How to Access Our Supportive Care Programs

Most of our programs have specific insurance and clinical criteria. To access our services, you may need to ask your physician for a referral or apply through the State of Vermont.

The best way to find out if your loved one is eligible for our programs is to call us at 802-658-1900.

Our friendly staff can help determine if you qualify and let you know how to apply. We can also direct you to other community resources as needed.
