UVM Health Cares provides personalized care navigation and support to all UVM Health employees and their families covered under one of our medical insurance plans.

Whether you’re managing a chronic condition, recovering from a hospital stay or trying to find the right primary care provider, the UVM Health Cares team is here to support you and your family — confidentially and at no cost.

When you connect with UVM Health Cares, you’ll have a dedicated team that includes a nurse, a care guide, a clinical pharmacist and a health coach.

Care Navigation

We can help you access care, schedule visits and maximize health benefits.

  • Enroll with a primary care provider.
  • Set up MyChart, your online patient portal.
  • Schedule appointments, referrals and preventive visits.
  • Support in using your health benefits effectively.
Transitions of Care

We can help ensure a smooth recovery after an emergency visit or hospital stay.

  • Review post-hospital instructions.
  • Schedule follow-up visits and coordinate ongoing care.
  • Review medications for safety and provide education and adherence support.
  • Identify and support potential recovery barriers such as transportation or home care needs.
Chronic Condition Management

We can help you manage ongoing health needs.

Managing Complex Needs

We can help you manage multiple providers, treatments and resources.

  • Provide health evaluations for personalized care plans.
  • Act as a central point of contact, coordinating referrals and follow-ups to prevent delays.
  • Ensure you understand and follow treatment and medication plans.
  • Care coordination during medical leave and return-to-work planning.
Support for Pregnant and Adoptive Parents

We can connect you with prenatal and postpartum benefits through our Baby Steps program.

  • Provide personalized guidance during pregnancy and after your baby arrives.
  • Assist with care coordination and referrals.
  • Access a free breast pump (available through Cigna).
  • Provide up to $350 in reimbursements for:
    • Childbirth classes
    • Doula services
    • Car seats, fitness support or in-home help
    • Books and other supplies
  • Available to adoptive parents of infants under age one
