Newly graduated advanced practice professionals (APPs) face many challenges as they transition from academic preparation into independent clinical practice, including role transition, high clinical complexity, increasing autonomy and accountability for high-acuity patient care.
Champlain Valley Physician Hospital's Advanced Practice Professional Transition of Practice (TOP) Program provides new graduate APPs with a structured, supportive bridge from academic training to confident, independent clinical practice — a need that is especially critical in rural and medically underserved communities.
APP TOP combines longitudinal clinical preceptorship, high-fidelity simulation, interprofessional education and reflective practice to build clinical competence, professional confidence and resilience.
The program strengthens patient safety, supports provider well‑being and retention, and promotes health equity by offering hands-on supervised care, case-based bedside teaching, advanced simulation training and expert-led education that meets national standards. This approach ensures our community receives skilled, compassionate and culturally responsive care.

The APP TOP Program was developed in alignment with national best practices and accreditation standards and is modeled alongside our medical, nursing and pharmacy residency programs to ensure a consistent, high-quality onboarding experience across disciplines.
