Transferring a Patient Within the UVM Med Center

Patient Logistics Services is for use by providers and their staff to arrange all patient transfers within the UVM Medical Center. A team of specially trained nurses assistin arranging patient transfers, including help with transport services.
Sharing Information Between Providers
Health care providers (primary care physicians, specialists, hospitals, clinical labs, etc.) can share information through a secure network using the information in the Direct Address List of Providers. Though the Direct address looks like an email address, it used to send and receive secure information into an electronic health record (EHR). It cannot be used by patients and their families to send messages to physicians, specialists, etc. Our primary current use is to receive referrals from community providers and to send Transition of Care reports to Primary Care offices.
