Our community programs are designed to make healthier living easier by addressing factors that affect the health and wellness of our community beyond our four walls.
Community Resources
At University of Vermont Health - Home Health & Hospice, our mission is deeply rooted in the communities we serve. Every day, we provide compassionate care and essential services to individuals and families across our region. We recognize that many of those we care for also benefit from the support of other local organizations.
That’s why we welcome and appreciate the strong partnerships and collaborations we’ve built with community groups and service providers. Together, we work to ensure that each person receives the most comprehensive, holistic care possible.
In addition to those mentioned below, we partner with many local health and social service organizations to support the health and well-being of residents of Chittenden and Grand Isle Counties and beyond. These include:
- Other home health and hospice agencies in Vermont to accept patients at McClure Miller Respite House.
- Nursing homes, schools and childcare providers, as our home health teams provide care in those settings.
Home Health & Hospice often relies on the support of local EMS and public safety organizations as we work together in neighborhoods, town and communities.
C.I.D.E.R. provides transportation and non-medical support to Home Health & Hospice clients and patients residing in Grand Isle County.
Our Longitudinal Care Program staff partner with CHCB to serve patients in CHCB’s Elder Care Doctor-at-Home service. We share many patients with CHCB and appreciate collaborating to ensure our patients have access to community resources.
CVOEO provides housing stability and support for Home Health & Hospice clients.
We partner with the Howard Center to provide case management in support of their clients’ mental health and substance use needs.
We provide nursing services for preventative care and wellness support for SASH® participants in Burlington and Winooski.
SSTA provides rides to our Adult Day participants and many Home Health & Hospice clients.
We coordinate with University of Vermont Medical Center to seamlessly transition patients between the hospital and their homes to ensure consistent, appropriate care.
Home Health & Hospice is a teaching site for nursing and therapy students, residents and fellows at UVM College of Nursing and Health Sciences and UVM Larner College of Medicine.
We are a member of VNAs of Vermont and partner with this professional trade association to advocate for legislation that supports home health and hospice care for Vermonters.
