Accountable Community for Health – THRIVE

We are proud to lead and support THRIVE, our Accountable Community for Health (ACH). Thrive brings together community partners dedicated to enhancing health and social services in Washington and Orange counties. Its goal is to make the community healthier and happier by working together, using trustworthy information, and trying new ideas.

Central Vermont Home Health and Hospice

Central Vermont Home Health and Hospice (CVHHH) is a full service, not-for-profit visiting nurse association. It is committed to providing high-quality, medically necessary home health and hospice care, whether someone can pay or not. CVHHH also provides maternal-child health services and long term care services.

Central Vermont Prevention Coalition

As the coordinator of the Central Vermont Prevention Coalition, we promote a community-wide hub-and-spoke approach to support people grappling with substance use disorders.

Family Center of Washington County

Family Center of Washington County offers a wide range of services, including childcare, parent education and family support to strengthen families and caregivers.

Green Mountain United Way

Green Mountain United Way is dedicated to building stronger communities in the counties of Caledonia, Essex, Orange, Orleans, and Washington. By focusing on health, education, and financial stability, it works with community partners to address and solve local challenges.

People's Health and Wellness Clinic

People’s Health and Wellness Clinic provides free essential health care to adults who lack adequate health insurance. Services include medical, dental, mental health and preventive health education.

Turning Point Center of Central Vermont

Turning Point Center of Central Vermont provides peer-based recovery supports, educational programs, and a safe haven for sober recreation and social activities. Its mission is to help individuals and families start, keep and improve their substance abuse recovery.

Vermont Department of Health

The Vermont Department of Health works to keep all Vermonters healthy in body, mind and community. It aims to ensure everyone has fair access to health care, prevent illness and injury, and get ready for health emergencies.

Washington County Mental Health Services

Washington County Mental Health Services helps people with disabilities, mental health issues and substance use problems. It provides crisis help, therapy and community support to help people improve their quality of life.
