Quality Assurance & Improvement Plans
Elizabethtown Community Hospital is committed to providing EMS Agencies within their catchment area with a non-punitive educational program designed to improve patient care. This will be based on local data and current best practices within EMS. There will be quarterly meetings to present this information.
QA/QI Meetings
QA/QI Meetings will be held the last Monday of the month for the prior quarter. Below is the list of dates for the meetings and due dates for requested for PCR information from each squad.
Agency Point of Contact: Please update July annually and as needed
- October 27, 2025 (Due date October 15)
- January 26, 2026 (Due date January 15, 2025)
Any EMS Agency is welcome to attend the QA/QI meetings. Collected data from regional agency's PCR's will be presented.

Cardiac Arrest, Major Trauma, Possible Strokes, Controlled Substance Use, Aeromedical Utilization
Any agency that would like an independent review of a call, would like follow up on a specific call or has quality concerns may email the QA/QI coordinator or their liaison.

Controlled Substances Forms
Forms are available to assist with agency certifications. All agencies are responsible to provide the pharmacy with the forms below. Additionally, once an agency is certified, they must provide the pharmacy with a copy of their 03C license.
