The Foundation of Champlain Valley Physicians Hospital offers grants, scholarships and funds to support our patients and their families, our employees and the next generation of health care professionals.

Strengthening Our Community

Our work at The Foundation of Champlain Valley Physicians Hospital involves providing a variety of grants and scholarships. Through these programs, we aim to help families, strengthen our workforce and improve the health of our community.

Chelsea’s Rainbow Fund

Chelsea’s Rainbow Fund helps families of children with cancer who are receiving care out of the area. The Kinblom family established this fund in memory of their daughter, who inspired hope, courage and love for others.

A committee appointed by The Foundation of Champlain Valley Physicians Hospital evaluates and approves all applications based on the following criteria:

  • Financial need: This is the primary criterion considered. In general, the maximum annual household income to receive a grant is $75,000. However, if you are over this threshold, please reach out to our office for exemptions based on special circumstances.
  • Age: Children must be 26 years of age and under.
  • Residence: Applicants must live in Clinton, Essex or Franklin County.

The fund provides financial assistance to the family and grant size is limited. We award grants on an equal opportunity basis regardless of sex, color, race, creed or religion.

Jane’s Fight Fund

Jane's Fight Fund was established in 2019 by the Sample family in honor of Jane Sample, who was diagnosed in November 2016 with Stage 4 stomach cancer. The fund helps ease financial burdens and lift the spirits of families in need by holding give aways three times a year. Recipients may use this gift as they see fit.

To qualify for a drawing, you must currently be undergoing cancer treatment and reside in Clinton, Franklin or Essex County, NY. Qualifying individuals must complete an application for entry into each drawing. For the drawing, they will be assigned a number to help with confidentiality. Drawings are held on June 30 (Jane's Birthday), October 30 and February 28.

Community Grant Funding Program

In keeping with the mission of The Foundation of Champlain Valley Physicians Hospital to improve the health of the community, the Board of Directors established our Community Grant Funding Program. We provide periodic grants to assist local nonprofit organizations addressing significant regional health and wellness issues in Clinton, Essex and Franklin Counties.

A committee of volunteers representing a cross‐section of the community established the guidelines and criteria for the grant program. The committee considers programs that address health-related issues. Projects showing cooperation and collaboration with other agencies are encouraged.

We award grants twice per year, spring and fall, and look for the following factors when reviewing grant proposals:

  • Addressing community health needs as defined in the community health assessment
  • Reliability, past record and the mission of the organization
  • Financial feasibility of the project
  • Pursuit of multiple funding sources
  • Partial funding request
  • Request for a specific project versus general operating funding
  • Projects that are not duplicative of another service in the community

All funding requests must include a completed Community Grant Funding Application and project budget. Applications must be completed through our online form which will be available when we are accepting applications. Please check back in the spring.

The Wish Fund

The Wish Fund grants wishes to people with a health condition or illness that limits their life expectancy. Applicants must be a resident of Clinton, Essex or Franklin County. The Foundation of Champlain Valley Physicians Hospital created this fund through a donation by the Adirondack Champlain Valley Realtors (ACVR).

ACVR partners with The Foundation to help manage the funds and application process. Together, we do our best to grant wishes for people unable to fulfill them on their own.

To apply, complete the application below, sign section 6 and mail the application to The Foundation of CVPH, 75 Beekman Street, Plattsburgh, NY 12901. We will review your application to determine your (or your dependent’s) eligibility and approve or deny your request as quickly as possible. If approved, we provide grants up to $5,000.   • Family trip to Walt Disney World
  • Car trip to Dollywood
  • Visit to the Bronx Zoo
  • Tickets to the Boston Bruins
  • A special family dinner
  • A pig roast with family and friends
  • Driving vacation around Maine in the summer

Travel Funds

Each travel fund was created with a specific purpose and has a unique history. Brief descriptions are below. We are happy to provide you with more information.

  • April LaValley Fund: Assists patients diagnosed with cystic fibrosis who must receive care out of the area. April won the hearts of North Country residents and was dubbed an ‘angel.’ Her wish was to help others suffering from cystic fibrosis.
  • CVPH Travel Fund: Assists adults who need specialized medical care out of the area. The Foundation of Champlain Valley Physicians Hospital Board of Directors established this fund in 2001.
  • Melissa Lahtinen Penfield Fund: Provides assistance to organ transplant recipients and donors. WIRY and the Lahtinen and Penfield families created this fund to honor Melissa’s memory and legacy.
  • Roger Senecal Fund: Assists income-eligible families who have ill children and need to receive care out of the area. The outpouring of support that Roger received as a little boy in the 1980s continues to make an impact today.

These funds are intended to provide assistance to the patient and one travel companion. In addition:

  • Funds may assist with gas mileage, ferry tickets, lodging and food
  • Funds may not assist with healthcare expenses, medications or medical equipment
  • Reimbursement for meals does not include alcohol and may not exceed $25 per day
  • Receipts and current mailing address must accompany all requests for fund reimbursement
  • Reimbursement is only for expenses outside the area
  • Grant size is limited
  • We award grants on an equal opportunity basis regardless of sex, color, race, creed or religion

Foundation Scholarships

The Foundation of Champlain Valley Physicians Hospital offers a number of scholarships to support education and workforce development.

High School Scholarships

High School Scholarships are available to graduating high school seniors from Clinton and Essex Counties who have chosen a degree in a health care-related field, such as:

  • Emergency medical services
  • Nursing
  • Pharmacy
  • Radiology
  • Rehabilitation therapy (physical therapy, occupational therapy, speech therapy)

Completed applications must be received by the Scholarship Committee no later than April 14, 2026. Applicants should attach a copy of their high school transcript and an essay (100 to 200 words) on why they are pursuing a degree in the healthcare field.

Champlain Valley Physicians Hospital Employee Scholarships

Educational grants are available to any CVPH employee seeking financial assistance to strengthen their role at CVPH by:

  • Pursuing an undergraduate or graduate degree
  • Obtaining a certification
  • Attending a conference

The Foundation of Champlain Valley Physicians Hospital also awards other scholarships annually through a variety of channels:

  • CVPH Nursing Scholarship: Supports CVPH nurses going back to school to further their education.
  • Kevin J. Carroll Scholarship: Provides educational assistance to CVPH employees seeking first-level certification or licensure in a health care related field.
  • Karen H. Prevo Memorial Scholarship: Assists a CVPH employee attending nursing school with a GPA of 3.0 or higher and who loves the field of nursing as Karen did.
  • Pearl Hayes Douglas Scholarship: Provides a $500 scholarship to a CVPH Patient Services staff member who is working towards the next step in their professional training.
  • Stephanie Wheet Fund: Awards a scholarship to a deserving CVPH radiology school second-year student.
  • Adam Jarvis Scholarship: Awards a scholarship to a deserving CVPH radiology school second-year student.
  • John Weldon Scholarship: Assists students pursuing a nursing degree at Clinton Community College.
