University of Vermont Health Affiliate Agreements with insurers have delegated provider credentialing to University of Vermont Health Credentialing & Enrollment. UVM Health Credentialing & Enrollment is certified by the National Committee for Quality Assurance in Credentialing.
UVM Health Credentialing & Enrollment Credentials Plan is intended to ensure a high quality, responsive network of practitioners who deliver care and meet the special needs and cultural preferences of the population they serve, to establish practitioner eligibility criteria for contracted pay or network participation conducted in a nondiscriminatory manner, and for insurers and practitioners to ensure that patients receive quality care and services from qualified professionals. The Credentials Plan and its Policies are approved by the UVM Health Credentialing & Enrollment Credentials Committee and are available for review herein.

To request a medical staff application, please email UVMHNCredentialing@UVMHealth.org. In the subject line, please indicate “Request for Intake Form”. A member of our credentialing office will respond with an intake form for your completion. Upon submission of the completed intake form, an invitation with a link to an online application will be sent to you for completion.
