University of Vermont Health Network Credentialing & Enrollment Plan and Policies

The University of Vermont Health Network Affiliate Agreements with insurers have delegated provider credentialing to University of Vermont Health Network Credentialing & Enrollment. The UVM Health Network Credentialing & Enrollment is certified by the National Committee for Quality Assurance in Credentialing.

The UVM Health Network Credentialing & Enrollment Credentials Plan is intended to ensure a high quality, responsive network of practitioners who deliver care and meet the special needs and cultural preferences of the population they serve, to establish practitioner eligibility criteria for contracted pay or network participation conducted in a nondiscriminatory manner, and for insurers and practitioners to ensure that patients receive quality care and services from qualified professionals. The Credentials Plan and its Policies are approved by the The UVM Health Network Credentialing & Enrollment Credentials Committee and are available for review herein.
