Clinical Services
We offer a range of services to meet our residents’ medical needs. Our location within Champlain Valley Physicians Hospital and our affiliation with University of Vermont Health ensure easy access to specialty care when necessary.
Our services include:
We can assist you with maintaining an appropriate medication schedule and administering your medications (including long-term antibiotics). Our nurses can communicate with your doctors if you need medication changes.
Our dietitian can work with you to create customized nutrition plans to help you gain or lose weight and improve your health.
Occupational therapists offer exercises and activities designed to help you participate in activities of daily living, such as tying your shoes or preparing a meal. We can help you adapt to your environment and teach special techniques that allow you to achieve your highest level of independence.
Physical therapy can help you decrease pain and maximize function. Our physical therapists recommend specialized exercises and activities to improve strength, endurance, flexibility and balance.
Speech therapists use the latest technologies and approaches to help improve communication, cognition or swallowing difficulties.
Our providers offer full-spectrum wound care for residents with slow-healing wounds from conditions such as diabetes or peripheral arterial disease.
After hospitalization for orthopedic surgery or other procedures, you may need skilled nursing and rehabilitative care that can’t be provided at home or in an outpatient facility. Our subacute rehab program can help ease the transition from hospital to home.
Our physicians, nurses, rehabilitation specialists (including physical, occupational and speech therapists), pharmacists and case managers work as a team to develop and implement a personalized care plan so you can recover safely and regain your independence.
