About Our Location

Whether you're having a major surgery or a routine procedure, health care can often be overwhelming. University of Vermont Health – Porter Medical Center offers case management and social work services to help you and your family cope with illness or injury.

Our team consists of nurses, social workers, utilization review assistants (who work directly with insurance companies to ensure payment for care provided) and case aides with expertise in a broad range of health care-related issues. To get more information or request support services, please call our Care Management staff between 8:00 am – 4:30 pm, Monday – Friday at 802-388-4778. After 4:30 pm and on the weekends, please call 802-388-4701 and ask for the social worker on call.
