Keeping track of your weight

How do I keep track of my weight?
You can use a chart to help you keep track (form 1).
Every time you weigh yourself, write down:
The date and time
Your weight
Important details you want to remember – For example, you can note what you ate, if you took your medicine, and how you were feeling.
Your doctor or nurse can tell you:
How and when to weigh yourself
When to take an action, such as calling the doctor, based on changes in your weight

Keep this chart near the scale. Each time you weigh yourself, write down the date/time and your weight. If there are other important details you want to remember, write them in the notes section. For example, you can note what you ate, whether you took your medicine, and how you were feeling.
