University of Vermont Health – Elizabethtown Community Hospital wants to make it as easy as possible for you to access your medical records, no matter where you receive care.
For many patients, portions of your medical record are available to view and print through MyChart. MyChart cannot be used to send medical records to other health care providers.

How to Request Medical Records

Records sent to another health care provider or facility are not subject to any fees.
