About Our Location
University of Vermont Health – Central Vermont Medical Center wants to make it as easy as possible for you to access your medical records, no matter where you receive care.
For many patients, portions of your medical record are available to view and print through MyChart. MyChart cannot be used to send medical records to other health care providers.
Existing users can access their records by logging in to MyChart. New users can sign up for a MyChart account. If you need help downloading your medical records, please read the MyChart Help Guide.
How to Request Medical Records
To request any portion of your medical record from Central Vermont Medical Center, complete a written request at the Medical Records Office at 3 Home Farm Way in Montpelier. If you are unable to come to the office, we will also accept a printed and signed authorization form.
Please note exactly what information you need. Most people only need a portion of their medical record, but you can obtain your entire record if needed.
Reasonable cost-based fees may be imposed for copies of your record. All fees charged shall be in accordance with applicable federal and state laws.
To send any portion of your medical record to another provider, please complete a written request at the Medical Records Office at 3 Home Farm Way in Montpelier. If you are unable to come to the office, we will also accept a printed and signed authorization form indicating where you’d like us to send the records.
Records sent to another health care provider or facility are not subject to any fees.
To request copies of your radiology images, such as X-rays, CT scans or MRI films, use the Radiology Image Record Request form.
