To request an application for medical staff appointment or privileges at any UVM Health Network Hospital, please send an email to UVMHNCredentialingUVMHealth [dot] orgdata-auth="NotApplicable" data-linkindex="0" id="LPlnk268128" rel="noopener noreferrer" target="_blank". In the subject line, please indicate “Request for Intake Form”. A member of our credentialing office will respond with an intake form for your completion. Upon submission of the completed intake form, an invitation with a link to an online application will be sent to you for completion.
