The University of Vermont Medical Center follows eligibility standards set by the National Institutes of Health (NIH) to determine bariatric surgery qualifications. NIH mandates an eligible candidate have body mass index (BMI) of at least 40, or a BMI of 35 with pre-existing co-morbid conditions, such as diabetes, obstructive sleep apnea or hypertension. Calculate your BMI (Body Mass Index).

Below is a brief outline of the steps involved in applying to the UVM Medical Center Bariatric Surgery program.

Step One: Request, Complete, and Return a Health Questionnaire

Step Two: Attend an Introduction Session

Once our staff receives and reviews your questionnaire, they will invite you to attend a two-hour Introduction Session. The session will be held at the UVM Medical Center Bariatric Clinic in Williston. You will learn about our current surgical options and receive information about our program requirements.

If you cannot attend the class in-person, we do have an online version. Please ask about this when you are contacted and invited to the Introduction Session.

Step Three: Complete a Psychological Evaluation

After you have completed the two-hour introductory session, you will need to meet with our team psychologist. This appointment is to make sure that you are ready for surgery and have the necessary support in place to meet the requirements of the program.

Step Four: Meet with a Surgeon, Dietitian and Physician Assistant

You will have monthly visits to the Bariatric Clinic for six consecutive months. You will meet with a surgeon or physician assistant and the dietician at each visit. The dietician will set an individual weight loss goal for you and follow your progress toward that goal. You will also be asked to attend a nutrition class, behavioral skills class, and one support group meeting prior to surgery.
