The University of Vermont Medical Center’s mission is to improve the health of the people in the communities we serve by integrating patient care, education, and research in a caring environment.

The UVM Medical Center:

  • Is one of 138 academic medical centers in the country
  • Is the only tertiary care Level 1 Trauma Center in Vermont (along with the state’s only Neonatal Intensive Care Unit)
  • Is a regional referral center for over 1 million people in Vermont and Northern New York
  • Is a community hospital for approximately 168,000, with 620 licensed beds
  • Employs over 8,800 people

Milton, Vermont is a relatively small community with just over 10,000 residents. It is located approximately 15 miles north of Burlington, VT and is home to Family Medicine Milton (also known as Milton Family Practice, or MFP) – the home of the University of Vermont Family Medicine Residency Program.

Family Medicine Milton is the continuity practice site for the Residency Program. It is dedicated to the education of our Family Medicine residents, and to the care of our patients. Our faculty and staff have extensive longevity at the practice, and it is truly a supportive, family-like environment. The practice is a NCQA-Certified, Level 1 Medical Home, and has well over 2,000 patient visits per month. Our clinic has been identified as repeatedly receiving the highest number of outpatient phone calls of all clinic sites in the network! (Frequently, the only department that supersedes the clinic is Room Service).

The 11,500 square foot building has:

  • 21 exam rooms
  • 2 dedicated treatment rooms for procedures
  • A patient counseling room
  • A small group therapy room
  • A conference room
  • On-site radiography
  • Office lab and on-site phlebotomy

The practice has a desk carrel for each of our 18 residents, with ample personal storage space, bathroom with shower, and a large well-equipped staff kitchen.

Family Medicine Milton is currently identified to be one of the pilot sites for our High Value Care Project.

We are working hard on the logistics of this, and more information will be provided soon!

At the practice, you will work closely with experienced, board-certified Family Physicians. These faculty members are available for consultation during your practice hours. They will offer you expert guidance as you care for patients with a comprehensive range of health concerns.

Family Medicine Milton reflects the type of group practice that many residents choose after completing their residency. Emphasis is placed on accurate problem-oriented records to enhance communication, appropriate use of nursing and counseling professionals, and direct involvement in the organization and administration of the practice.

Patients often comment that they prefer to have all of their care at MFP, as opposed to driving into the “big city” of Burlington for specialty care. This lends itself to the provision of full-scope family medicine on site, including:

  • Behavioral health counseling
  • Social work
  • Nutritional counseling
  • Clinical pharmacist support

Family Medicine faculty and residents perform many procedures embedded in their continuity clinics.

These procedures include, but are not limited to:

  • Laceration repair
  • Skin biopsies
  • Casting and splinting
  • IUD and LARC placement and removal
  • Join injections
  • Colposcopy
  • Vasectomy
  • Nail procedures

Technology highlights include:

  • The same medical record system in both the inpatient and outpatient setting (EPIC)
  • Telemedicine capability (both audio and video)
  • Medical pharmacologic and coding database
  • Medical literature search capabilities

The following Family Medicine sites provide a diverse group of attending clinical faculty with a wide range of interests. Medical students also rotate through these practices. Faculty from the practices teach residents at the Milton site as well as on the Family Medicine inpatient service. Additionally, our residents complete a rotation at our Urgent Care Center which is staffed by Family Medicine physicians.

The Community Health Centers of Burlington (CHCB) is a Federally Qualified Health Center, located within Burlington proper. It has multiple sites within Burlington with many sites geared toward specific outreach. For example, there is the Pearl St Youth Clinic, Safe Harbor Homeless Health Clinic, school-based dental care, and as well as a continuity clinic with an emphasis on obstetrics (including obstetrical ultrasound, prenatal/ postnatal care), pediatrics, and care of New American/ refugee families. The CHCB is staffed primarily with Family Medicine physicians.

Our residents have the opportunity to work closely with the CHCB throughout various rotations. We admit CHCB patients to our inpatient service, we deliver all CHCB patient babies, and we manage their newborn nursery care, all with their FM attendings as well as their midwives.
