Grace Zimmer is a family nurse practitioner at the University of Vermont Medical Center. She specializes in women’s health
and pediatric care.
Grace’s philosophy of care is rooted in the idea of partnership. In a partnership with her patients she strives to provide care by forming a trusting relationship, and establishing open, honest communication. She believes in this partnership she and her patients can work together to achieve the common goals such as infection treatment, managing chronic conditions, wellness promotion, and disease prevention. She tries to put herself in the shoes of her patients by speaking to them like she would like to be spoken to. Grace takes every possible step to explain the plan of care, treatment options, expected outcome, and any complications that might arise. By doing this, she is able to provide patients with the tools they need to take an active part in their health care. She believes health decisions that are made together have the best chance of success.
