Job Description

This position is for an Ambulatory Pharmacist Clinician embedded in the Rheumatology and Dermatology practices at Central Vermont Medical Center (CVMC). This position will work with the UVMMC Specialty Pharmacy team to integrate clinical and operational pharmacy services for specialty pharmacy medications.


The OP Ambulatory Pharmacist Clinician is an advanced practice pharmacist responsible for assuring optimal drug use outcomes in high risk, high cost or complex patient populations. She/He applies specialized knowledge of pharmacotherapy to direct patient care, teaching, and consultation. Participates as a contributing member of ambulatory care teams such as rheumatology, neurology, infectious disease, oncology, nephrology, or primary care. Provides clinical, pharmacy, and medication expertise in each area including policy development, pharmacy operations, prior authorization support and medication therapy outcomes.


Doctor of Pharmacy and an ASHP accredited residency program or equivalent experience preferred. Licensed pharmacist in the State of Vermont.


3 years of advanced practice experience in their area of expertise preferred or equivalent experience in ambulatory care or a clinical outpatient setting.

*Conditional offers of employment in the Specialty Pharmacy department are contingent upon favorable completion of a thorough background check and post-offer, pre-employment drug screening. Candidates must submit to a drug screen from a vendor approved by the UVM Medical Center.  Specialty Pharmacy candidates who refuse to obtain a drug screen within the time frame requested may have their offer of employment rescinded.  If a Specialty Pharmacy candidate’s drug screen reveals a positive test result, dependent on the circumstances, his/her offer of employment may be rescinded.
