Job Description
External candidates with one year of LPN experience are eligible for a one-time signing bonus of $7,500 (prorated for part time roles). The bonus will be paid out as a $3,750 gross pay adjustment in the first pay check; the second installment of $3,750 will be paid upon completion of one year (2,080 hours/prorated for part time) of service in the department hired into. Amounts reflect gross pay, prior to applicable tax withholdings and deductions required by law.

JOB DESCRIPTION:
The Ambulatory LPN – Primary Care is an integral part of the care team and a key contributor to delivering high quality care that is compassionate, consistent and reliable for our patients, families and the practices within a medical home. The incumbent is involved in direct patient care under the direction of a provider, provides technical nursing care to patients in varying states of health and illness and performs clinical and administrative health care support services to patients and families according to knowledge, ability, and duties assigned.
EDUCATION:
Current LPN licensure or compact licensure recognized by the State of Vermont required. CPR certification required within the first six months from hire.
EXPERIENCE:
One to two years experience in relevant health care setting or training program desired. Primary Care experience preferred. Proven customer service experience required.
