Want to be healthier and feel better? Our local coordinator can connect you with free evidenced based workshops in your community. These are led by trained facilitators who know first hand what it means to live with chronic health conditions and have faced their own health challenges. Or they support someone living a chronic health condition. These programs focus on supporting sustainable behavior change. Whether the goal is to prevent type 2 diabetes, manage blood pressure, be nicotine free, manage chronic pain or other long-term health problems, Vermonters who complete our workshops see big improvements in their health— and their overall well-being, while working toward their own health and wellness goals.

There are six workshops available to Vermonters and anyone who receives their health care from a Vermont provider. These workshops are:

  • Diabetes Prevention Program
  • Hypertension Control Program
  • Diabetes Self-Management
  • Chronic Pain Self-Management
  • Chronic Disease Self-Management
  • Tobacco Cessation Workshop

Small groups of 4 to 15 participants meet weekly with trained facilitators online in a ZOOM Meeting or at locations throughout Vermont. Workshop lengths and times vary.

We welcome family members and friends who support participants to attend the workshops too.

Workshops topics include:

  • Healthy eating
  • Weight management
  • Stress management
  • Communicating with family and health professionals
  • Problem-solving and goal setting
  • Getting a good night's sleep
  • Medication usage
  • Nicotine replacement therapy usage
  • Dealing with triggers and withdrawal
