The Policy covers those web pages with a "uvmhealth.org" or "medicalcenter.uvmhealth.org" Internet address (the Internet address is typically displayed at the top of your browser).
This Policy is provided for the benefit of the general public. The Policy does not apply to information submitted through the Web site by others, such as health care providers, residents, or applicants for residency or fellowship programs, when acting as such.
What Personal Information Do We Collect?
You may use this Web site without providing any personal information (e.g., your name, address, or phone number). We only obtain personal information if you choose to provide it to us. The three items identified below are examples of situations where you can submit personal information through the web site.
We collect personal information if you submit, through the Web site, a:
- question to our Webmaster;
- Comprehensive Health Enhancement Support System ("CHESS") participation form; or
- resume or application for a job.
How Do We Use Personal Information?
No Sales of Personal Information. We do not sell any personal information.
Questions. You may, via e-mail, submit an inquiry to our Webmaster. The e-mail will contain your e-mail address and may contain other personal identifiers. We may, depending on the context, respond to your e-mail and we may retain your e-mail in our files.
CHESS Forms. Programs available through CHESS require that you submit your name, e-mail address, telephone number, and mailing address on a registration form. We use this information to verify your participation in a program. We do not share that information with any third party. We do share some demographic information you submit on the registration form, such as your gender and age, with the University of Wisconsin at Madison, the entity that provides the programs through CHESS.
Job Applications. If you apply for a job through the Web site you will complete a form which requires, among other things, your name, mailing address, telephone number, and resume. The information you submit is stored in a database maintained by our Human Resources Department and in a separate database, for back-up purposes.
How Can You Remove Personal Information From Our Files?
You can have us remove personal information from our files by sending an e-mail to our Webmaster at email@example.com. In the e-mail, please provide your name and identify the activity in which you provided the personal information. We will honor your requests in all instances where we are reasonably able to do so, though removing the information may lead to your release from a program or other offering (such as with a CHESS program).
What Can You Assume About Links to Third Party Web Sites?
Our Web site does contain links to other sites, but this Policy does not apply to those sites. As noted earlier, this Policy applies only to web pages with a "uvmhealth.org" or "medicalcenter.uvmhealth.org" Internet address. We encourage you to read the privacy policies on other sites before providing any personal information through those sites.
Do We Track Your Use of the Web Site?
We collect the domain name of your computer server when you access the Web site, but we use that information only to determine how many persons visit our site on a daily basis. We also track all uses of the site to gain information to help us understand how you use the site, what portions of the site are most frequently accessed, and how we can improve the site to better meet your needs.
What About E-mails You Submit Through the Web Site?
You may submit e-mails through the Web site, and those e-mails will often contain your personal information. This Policy applies to e-mails you submit to our webmaster. The Policy does not apply to personal information you may include in e-mails that you submit to individuals other than the webmaster or directly to departments (e.g., the pathology department).
