Requesting Your Medical Records at the UVM Medical Center

To request a copy of your medical record, please print out the authorization to release patient information form and fax, mail or bring it to your physician's office. The contact information for the physician's office will appear on the document.

If the medical records you request total less than 25 pages, your records will be mailed with a bill. If the medical records you request are more than 25 pages, you will receive a pre-bill from the copy service that needs to be paid before the reproduction of the records.

If you request copies of your medical records to transfer your care out of the UVM Medical Center network to your new provider, there is a charge for the reproduction of the records. A flat fee of $5 will be charged for the first 10 pages; for every page over 10, the charge is .50 cents per page.

DISCLAIMER: Incomplete or preliminary information from the medical record will only be released in urgent or emergent situations and only for patient care. For all other requests, the medical records must be complete before any information will be released. Any exceptions to this rule need to be authorized by a member of the management in the Health Information Management Department or Case Management.

Access Your Medical Record Online

Request Radiology Images

To request copies of your radiology images, such as X-rays, CT scans or MRI films, contact the Imaging Library at 802-847-2354.
