Improving the Health of our Patient Population for More Than 25 Years

The Community Health Improvement (CHI) program at The University of Vermont Medical Center enables us to impact and serve thousands of people in our community with a wide variety of health, wellness and safety programs, including:

  • Adult Outreach program that provides foot care in the community
  • Community Benefits program which uses the Community Health Needs Assessment to link strategy and partnerships between UVMMC and the community to address health priorities through a variety of ways, including pediatric initiatives
  • Community Health Team - includes social workers, RN’s and clinicians embedded in primary care offices to support patients with various resources including Medication Assisted Treatment for people with Opioid Use Disorder ( remove previous link and add:

"Population health" is a buzzword used commonly in health care today that refers to proactive preventative care and wellness initiatives focus on reducing the need for intensive and costly health care later in life.

Our Community Health Improvement group has been a pioneer in this type of thinking, having been dedicated to exactly this for more than 25 years. This means that people in the state of Vermont and northern New York who are served by The UVM Medical Center have at their disposal tried and true programs focused on well-being that have only begun to be experimented with in other parts of the country.
