Diabetes Care at the UVM Medical Center

More than 26 million people in the United States are living with diabetes. Another 79 million Americans have prediabetes and are at risk for developing type 2 diabetes. Diabetes is a widespread and chronic disease which presents unique challenges to each person living with it. The University of Vermont Medical Center uses a team approach to offer comprehensive care for all types of diabetes. Our goal is to help you live well with diabetes and prevent or delay any diabetes-related complications.

Your Diabetes Care Team

The UVM Medical Center's team approach is specifically designed to meet your individual needs. We help with lifestyle modification, medication management and blood glucose monitoring, and our team works closely with your primary care provider to make sure you get the best care possible.

Our Living Well with Diabetes program is nationally accredited by the American Diabetes Association and is comprised of the following professionals and team members:

  • Board Certified Physicians and Nurse Practitioners offer extensive experience in the medical management of diabetes. Our physicians are board certified in Endocrinology (the area of medicine related to organs involved in diabetes) and are professors at the University of Vermont College of Medicine.  
  • Certified Diabetes Educators (CDE): A CDE is a registered nurse, registered dietitian, pharmacist or social worker with specialized training in diabetes education and care management. A CDE offers in-depth knowledge in nutrition, diabetes medications, communication, counseling, and education to help you successfully manage diabetes and avoid diabetes-related complications.
  • Registered dietitians: Registered dietitians at the UVM Medical Center provide personalized and culturally sensitive nutritional counseling. All of our dietitians are also Certified Diabetes Educators. They help you optimize your diabetes control with weight management strategies and medical nutrition therapy.  
  • Registered nurses: Our registered nurses skillfully address your questions and concerns and provide helpful resources. They are available during clinic hours to help you with:
    • Refilling prescriptions, including diabetes supplies
    • Finding community resources
    • Referrals to the UVM Medical Center Health Assistance Program
    • Scheduling of urgent appointments and referrals to other departments.
  • Certified Medical Assistants greet and escort you to the exam room. They check your vital signs and review your list of medications to make sure that everyone involved in your care is aware of all of your current medicines.
  • Receptionists greet you on arrival to our clinic. After your visit you will check out with one of our receptionists so that you can schedule any needed follow-up appointments and get your After Visit Summary.
  • We offer convenient lab services in our clinic. If your physician or nurse practitioner has recommended blood or urine tests we are able to collect those samples before you leave the clinic.

Preventing Diabetes

If you are at risk of type 2 diabetes, you may be able to reduce your risk or delay the onset of the disease by making the following lifestyle changes.

  • Make healthy food choices - Eating a healthy diet can help lower your risk for type 2 diabetes and other diseases.
  • Be physically active - Being physically active can help keep your blood glucose, blood pressure, cholesterol and triglycerides on target.
  • Maintain a healthy weight - Fatty tissues increase cell's resistance to insulin. Maintaining a healthy weight can help prevent the growth of fatty tissues.
  • Regular medical follow-up - Annual health checks which include diabetes screening are important in preventing or delaying the onset of type 2 diabetes.
