How to Participate

To include your project in this year’s Quality Forum there are two steps:

  1. Submit a Poster Reservation. This will enable us to hold a space for your poster at the DoubleTree by Hilton and will allow us to include your project in this year’s event program. Complete the Poster Reservation survey by March 16, 2018. A confirmation email along with the Poster Template (PowerPoint) will be emailed to you within 2 business days after receipt of your poster reservation.
  2. Complete the Poster Presentation Template. This will enable us to electronically send your poster for printing and lamination. Once completed, the poster will be returned to the Jeffords Institute for Quality and displayed for you at the Quality Forum. Following the event, your poster will be available to you. Please note that the Jeffords Institute for Quality will cover the costs associated with poster printing and lamination.  The Poster Presentation Template must be completed and sent to the Jeffords Institute for Quality at QualityForum@uvmhealth.org by April 13, 2018 to be included in this year’s Forum.
